Clinical Significance of Clostridium Difficile Isolated From Stools of Inpatients

|
Clostridium difficile is the most important cause of antibiotic-associated colitis. A total of 430 stool specimens from inpatients of Seoul National University Hospital were cultured on cycloserine-cefoxitin-fructose agar with 0.1% taurocholate for the isolation of C.difficile from November, 1990 to March, 1991. C.difficile could be isolated from 22 patients (5.1% of total specimens). 8 of 22 patients were found to have C. difficile-associated diarrhea as a result of retrospective studies of the clinical status. Most patients with C. difficile-induced diarrhea had cancers or were being treated after operation and showed diarrhea after administration of antibiotics for 6 to 22 days. Various drugs were administered, especially cephalosporins and aminoglycosides. In 5 of 8 patients with C.difficile-induced diarrhea, the isolation of C. difficile were requested because clinicians suspected antibiotic-associated diarrhea and so it seems to be necessary that all stool specimens obtained for bacterial cultures are processed routinely for the isolation of C. difficile in spite of the cost and time problems.
